Send a referral through the secure link, with the diagnosis, any precautions and what you want us to look at.

A call to the patient
Within one business day, our front desk calls to check coverage and set a first visit.

An evaluation
A physical therapist examines the patient and writes a plan of care, which we send to you for signature.

A note back
You hear from us at the evaluation, after any change, and when care ends.
What referring providers ask
Do you need a referral?
In Iowa, a patient can start without a referral. A plan may still ask for one, and we say so at the first call.
Can you work with our precautions?
Yes. Please include them in the referral, and we will follow them and tell you if anything changes.
Do you see people in senior communities?
We run classes and screening days in communities, and see people one-on-one in our clinic.
